The Rejoice Christian Athletic Department would like to thank you for taking part in our online submission process for athletic participation.

Note: You MUST have your student’s ID# before starting the online process. Also, make sure your student is present while completing the forms as they will have to sign each form.

1. To BEGIN, click on the DOWNLOAD AND PRINT tab. Click on the link to the OSSAA Physical Form and print the form (it cannot be filled out electronically). This is a two page document. The first page is to be filled out by hand and signed by student and parent. The second page is to be completed and signed by your doctor. Once completed, turned in to the athletic office.

2. NEXT, click on the ELECTRONIC PARTICIPATION FORMS tab. Individually click on each of the required forms (detailed on the HOME tab) and fill out all of the information requested. If a question is not applicable, please answer with “NA” or “UNKNOWN”.

3. To SIGN EACH DOCUMENT, click inside the signature box and hold your mouse down. This will allow you to create an “electronic signature”. If you make a mistake and need to start over, click the refresh icon next to the signature box. If using an iPad, your may sign by using your finger or stylus directly on the screen.

4. Once you have filled out all of the information on each form, you will have the opportunity to print the document(s).
